The original February 2026 methodology
The original public technical brief was titled Handoff Diagnostic - Operational Safety Layer for AI-Integrated Systems. It focused on failures that arise when decisions, information, responsibility, and authority move between humans, AI models, systems, and decision authorities.
Its core hypothesis was that operational integrity requires measuring the reliability of those transitions, not merely the quality of an isolated model output.
Handoff Integrity Score (HIS) v0.1
The February brief defined a 0-100 score composed of five 0-20 sub-scores:

The February worked example analyzed a compliance-review workflow and flagged Signal Degradation, a Decision Traceability gap, a Role/Authority mismatch, and an Override Health risk. The example returned an HIS of 62 with specific remediation steps.
The implementation brief described a structured capture layer, append-only audit-friendly storage, a scoring engine, a one-page audit report with recommended fixes, and optional governance thresholds that trigger review.
